Most terminally-ill patients and families are able to remain in the comfort and security of their own homes while receiving hospice care.
Under the direction of the patient's physician, a registered nurse coordinates the activities of the team as we bring these services to our patients in their homes.
Services Provided:
- Physician-directed care
- Registered nurse visits
- Hospice Care Team members are on call 24 hours a day, seven days a week
- Social Worker services for patient and family
- Home Health Aide
- Chaplain and spiritual assistance
- Volunteer support
- Diagnosis-related medications
- Medical equipment and supplies
- Respite care for patient or caregiver
- Supplemental nursing care in times of crisis
- Bereavement services
Hospice services are covered by Medicare or Medicaid, private insurance, private pay or charitable care. This care is provided to all in need of our services regardless of age, sex, race, creed, marital status, sexual preference, color, national origin, religion, citizenship status, veteran status, illness, handicap, disability or ability to pay.
It is possible that your loved one will improve, resulting in discharge from hospice services. Or, you may decide to stop hospice care because you believe aggressive, curative care is needed. The hospice services can easily be resumed if needed at a later time.
